This temple is located in Singarayapalem, Mudinapalli Mandalam of Krishna district in Andhra Pradesh. The worshipping deity is a five-headed snake. As per a story, a divine snake from Babaji Mattam visited Lakshmi Narasimha Swamy temple lake and took bath and while returning, was found by two farmers who hurled stones at it. The snake died by hitting its head to the ground. The villagers buried the body of this snake on the banks of Chavurupalemi Lake and those two villagers who threw stones lost their sight. Later when apologized to the snake, they got back their eyes. On the same night, the snake appeared in their dream and ordered them to construct a temple.
The following morning, the villagers excavated the body of the snake and took it in a procession to the surrounding four villages. They found a black cow (Kapila) coming out from its herd, going to the lake near Lakshmi Narasimha Swamy temple for a bath and went pradakshinas around the place. They started digging in this place, astonished to see the cow’s behavior and considered this place apt for building the temple bringing the snake’s body and performing the rituals. A temple was built here, previously called as Nageswara Swamy temple. The temple is now taken over by the endowment department of Andhra Pradesh. After renovation, the temple came to be called as Valli Devasena Sametha Subramanya Swamy.
People suffering from Naga Dosham for marriage perform Abhishekam and go pradakshinas for 5 Tuesdays, and it is proved that before the Pooja ends on the 5th Tuesday, they get married. Childless couples also have babies and they visit the temple after the child is born. Abhishekam ticket costs 50/-. People lately have seen a snake visiting this temple lake and they say snakes can be seen frequently. People here offer Pongal to the God and also they give their hair to the God here.

Sashti is celebrated in a grand manner for 13 days, on the Sasti day kalyanotsavam for Sri Vallidevasena Sametha Subrahmanya Swamy will be performed.
On Ekadasi laksha patri puja is celebrated.
On Navami day Rathotsavam is celebrated.
On Dashami day teppotsavam is celebrated in the pond near the temple. Teppotsavam in Subramanya Swamy will be performed only in the temple.
It is believed that childless people who worship Subramanya Swamy here with devotion and faith are blessed with pregnancy and pregnant women with safe deliveries.
Annaprasana, Namakaranam, Ear Boring Ceremony and Talaneelalu (Head Hair shaving) is also performed at this temple.
There is a gosala in the temple compound were gopujas were performed.
On sunday & Thurday huge number of devotes visit this temple.
On Tuesdays Dosha Nivarana Poojas like Kala Sarpa Dosham is to perform Rahu Ketu Pooja are also performed at this temple.
